DOT Exam Checklist for Drivers
Valid Driver’s License
Ensure you have your current driver's license or CDL with you.
List of Medications
Provide a complete list of all prescription and over-the-counter medications you're currently taking, including dosages and the contact information of the prescribing doctors.
Corrective Lenses or Hearing Aids
If you use glasses, contact lenses, or hearing aids, bring them along, as you'll need to pass vision and hearing tests.

Bring a letter from your cardiologist detailing your medical history and stating that you're fit to drive. Include recent test results like stress tests or echocardiograms.

Provide a letter from your neurologist outlining your medical history and current condition, confirming your ability to drive safely.

Bring recent International Normalized Ratio (INR) lab results and a letter from your doctor stating you're safe to drive
