DOT Exam Checklist for Drivers


Valid Driver’s License

Ensure you have your current driver's license or CDL with you.


List of Medications

Provide a complete list of all prescription and over-the-counter medications you're currently taking, including dosages and the contact information of the prescribing doctors.


Corrective Lenses or Hearing Aids

If you use glasses, contact lenses, or hearing aids, bring them along, as you'll need to pass vision and hearing tests.


Additional Documentation for Specific Health Conditions

Bring the most recent Hemoglobin A1c (HbA1c) test results and blood sugar logs.


Sleep Apnea

Provide a compliance report from your CPAP machine showing at least 4 hours of use per night, 70% of the time, over the past 30 days.


Cardiovascular Issues

Bring a letter from your cardiologist detailing your medical history and stating that you're fit to drive. Include recent test results like stress tests or echocardiograms.


Neurological Conditions

Provide a letter from your neurologist outlining your medical history and current condition, confirming your ability to drive safely.


Blood Thinners

Bring recent International Normalized Ratio (INR) lab results and a letter from your doctor stating you're safe to drive


Recent Surgeries or Illnesses

Provide clearance letters from your treating physician indicating you're fit to drive
